***New Jersey Department of Education has changed requirements for students to receive clearance to participate in Sports- PLEASE REVIEW ***

For each student to be eligible for the first practice: 

 A.    Complete electronic forms (numbered 1-5 on page 2).  These can be found after you register on the website: www.gotomysportsphysical.com.

 B.    Submit a physical that is less than 365 days old from the first day of official practice. Starting May 1, 2014, the new Pre Participation Physical Evaluation (PPE) form must be used for all students submitting physical examination for fall sports. If the annual physical was done prior to May 1, 2014, the old forms that were submitted and are on file in the health office can still be used.  After May 1, 2014, the new forms must be used.  These can be found on the SHSD website (Health Office Link and Athletic Link).  They are also available at http:www.state.nj.us/education/students/safety/health/records/athleticphysicalsform.pdf.

The New Jersey Department of Education state mandated forms include:

a.     History Form to be completed by the parent or guardian,

b.    Supplemental History Form for students with Special Needs to be completed by the parent or guardian.

c.     Physical examination Form and a Clearance Form to be completed by the examining physician, advanced practice nurse (APN) or physician’s assistant ( PA).

d.    If the pre participation physical examination was conducted 90 or more days prior to the first practice session of the athletic season, the parent or guardian of each student-athlete must complete and sign an updated health history questionnaire.  This Health History Update Questionnaire may be found at:  http://www.state.nj.us/education/students/safety/health/records/Heatlh History Update.pdf.

C.    Submit the $75 co-curricular fee to Mr. Hoppe’s office or via RevTrak (link on district website home page, click “Online Payments”).  Please include the Co-Curricular Participation Form to submit to Mr. Hoppe’s office (available for download on the athletics webpage).

Check Your Clearance Status by logging into gotomysportsphysical.com:

·         Click “View Profile for (Athlete’s Name)”

·         Status: Complete and Waiting for Approval = Forms being processed

·         Status: Complete and Stamped = Athlete is cleared for fall season

Please refer to instructions below for the NEW procedure to submit forms to be cleared for participation in sports. Please email our School Nurse, Anne Connor @aconnor@shsd.org or our Athletic Trainer, Matt Bale-Pena, at mbalepena@shsd.org with any questions.

 *****REGISTERING ON GOTOMYSPORTSPHYSICAL.COM: Registration opens June 3rd*****

      Go to the website: gotomysportsphysical.com

      Scroll down to the “Athletes/Participants” and “Register” or sign in with your email address

      “Register New Athlete” for each child participating (one account is used per household)

      Return to the “Home” screen and Select “Fill Forms”, choose athlete, select New Jersey, select Bernards HS, select sport, click “Initial Registration”, and click “Proceed”.

      Click “Start filling out a new form” for each of the 5 available forms.  Complete each form fully and click “Submit” at the bottom of the page for each of the 5 forms.  Click “Return to Registration” after submitting each form and proceed until all 5 forms show a status of “Completed”.

      ALL ATHLETES must complete the electronic forms listed below:

1. Interscholastic Sports Policy- with parent and student electronic signature

2. Permission for Participation- with parent electronic signature

3. NJSIAA Concussion Policy- with parent and student electronic signature

4. NJSIAA Steroid Policy- with parent and student electronic signature

5. Emergency Contact Information- with parent electronic signature

 New Jersey PREPARTICIPATION PHYSICAL EXAMINATION AND HEALTH HISTORY UPDATE QUESTIONNAIRE FORM

A.    A.  Pre Participation Physical Examination Form:  A physical that is less than 365 days old from the first day of the start practice must be submitted.  Print the 4 page Pre- Participation Physical Evaluation (PPE) Form found on the shsd.org webpage.  This four page form can be found on both the Athletic Link and the Health office Link.  Parent or Guardians should complete and sign both the History Form and The Athlete with Special Needs Supplemental History Form, and then have the athlete sign the both forms.  Bring them to the participant’s physical examination appointment with your treating physician.  The Physician will review the History and the Special Needs Supplemental Form, complete the Physical Examination Form and the Clearance Form during the exam for a physical administered within 365 days before first day of official practice.  Make sure all sections are complete.  Make sure physician stamp is next to physician signature.  Incomplete forms will be returned.  Staple all forms together and submit to the red box in the main lobby at BHS between June 3rd and July 15th

 B.    Health History Update Questionnaire Form:  In order to participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student’s parent or guardian.

 C.    Sudden Cardiac Death Pamphlet Sign-Off Form:  All student and parents must review the Sudden Cardiac Death in Young Athletes pamphlet that can be found on the SHSD website, or gotomysportsphysical.com website or at http://www.state.nj.us/education/students/safety/health/services/cardiac.pdf .  Students and Parents must complete and sign the Sudden Cardiac Death Pamphlet Sign-Off Sheet and submit it with physical and/or health history update questionnaire form.

If I may, a comment about officiating and sportsmanship. Good and bad officiating is part of all sports and at all levels. If professional officials cannot get it right all of time, it’s difficult to expect HS officials to get it right (all of the time). When they don’t, as a parent I know you want to scream, yell and voice your opinion. I’m asking that you refrain from doing this. It’s difficult enough to get officials to work our games and when parents lose control, it only makes matters worse. We’re the adults and for better or worse, it’s up to us as parents to set the example of appropriate behavior at athletic events. Former Acting Governor Richard Codey had legislation passed regarding fan behavior at athletic events. I think we do an outstanding job at BHS and I hope the positive behavior that we have exhibited in the past continues. Please review again the additional Sportsmanship information on the shsd web site.

NCAA Eligibility Academic Standards are changing!
  • Did you know that the NCAA’s initial-eligibility academic standards are changing for student-athletes enrolling at an NCAA Division I university on or after August 1, 2016 (this year’s high school sophomore class and younger)? A brief summary of the changes can be found here.
  • To assist in getting this message out, we have developed the Initial-Eligibility Resource Index, a new electronic tool that contains links to the NCAA Eligibility Center’s resources and presentations available free of charge.  
Key Reminders
  1. Additional high school-specific resources are available on the High School Portal Resources page. The NCAA Eligibility Center receives thousands of calls each year regarding hardcopy transcript receipt due to the two- to three-week processing time needed to mail, log, scan, and post transcripts to student accounts. You are strongly encouraged to submit transcripts through one of the approved e-transcript providers (Docufide/Parchment, Scrip Safe, ConnectEDU, National Transcript Center/Pearson Edustructure, USMO ET, and XAP) as these transcripts usually appear in student accounts within 48 hours.
  2. Please verify that your school’s List of NCAA Courses is up to date by logging into your account within the High School Portal . This will help ensure that courses taken by your students can be matched to their transcript during the final academic certification process.
  3. Catch us live at  www.collegeweeklive.com at 11 a.m. Eastern time Thursday, October 24. The free webinar can be found under the Events tab titled "All Access Fall event through CollegeWeekLive" and the name of the webinar is "Path to the Student-Athlete Experience."
Bus Transportation to Polo Grounds
Attention Parents and Athletes:

There will be two buses available after school to transport student athletes to the Polo Grounds for games and/or practices every day, if there are seats available. The buses will be the LAST TWO buses parked in the "bus line" in front of the school.
