ALL FORMS ARE DUE NO LATER THAN October 29, 2014

Please note that once documentation is submitted, it will be sent to our school physician every Tuesday, and will be returned the following Tuesday after delivery.  Complete clearance to participate in winter sports involves a two week turnaround time.  It is important to have all forms submitted by October 29, 2014 to allow time for athletic trainer, school nurse and school physician to review state mandated documents and communicate clearance to coaches and athletes.  Incomplete forms are not accepted and will be returned to the student.  All medical forms should be submitted to the RED SPORTS BOX, BHS MAIN OFFICE LOBBY.

For students to be eligible for the first practice

  1. Complete all electronic forms (numbered 1-6 on back of this memo).  These can be found after you register on the website:  www.gotomysportsphysical.com.  

  2. Submit the FOUR-page New Jersey Department of Education Sports Physical that is dated less than 365 days old from the first day of official practice. Starting May 1, 2014, the New Pre Participation Physical Evaluation (FOUR page PPPE) form must be used for all students requesting clearance to participate in winter sports.  If the annual physical was done prior to May 1, 2014, the old forms (Part A and Part B) that were submitted and are on file in the health office can be used.  Physical Examinations done after May 1, 2014 need the new forms.  These forms can be found on the SHSD website (Health office Link and Athletic Link).  They are also available at http: www.state.nj.us/education/students/safety/health/records/athleticphysicalsform.pdf.  

  3. If the sports physical on file is dated less than 365 days old but more than 90 days old from the official practice start date, you will ONLY need to submit a Health History Update Questionnaire (HHUQ) form.  

  4. Submit the $75 co-curricular fee to Mr. Hoppe’s office or via RevTrak (link on district website home page, click “Online Payments”).  

COMPLETE CLEARANCE REQUIRES TWO WEEKS FROM SUBMISSION OF DOCUMENTATION, SO IF YOU SUBMIT FORMS AFTER DUE DATE YOU RISK NOT BEING CLEARED FOR FIRST DAY OF PRACTICE.

Registration on www.gotomysportsphysical.com opens October 15th.

Check Your Clearance Status:

  1. Log into your account at www.gotomysportsphysical.com

  2. Click “View Profile for (Athletes Name)”

  3. If Status says Complete and Waiting for Approval that means your forms are being processed.

  4. If Status says Complete and Stamped that means athlete is cleared for winter season.

 

Please refer to instructions on back for the NEW procedure to submit forms to be cleared for participation in winter sports.    (OVER)

  • Go to the website: www.gotomysportsphysical.com

  • Scroll down to the “Athletes/Participants” and “Register” or sign in with your email address

  • “Register New Athlete” for each child participating (one account used per household)

  • Return to the “Home” screen and Select “Fill Forms”, chose athlete, select new Jersey, Select Bernards HS, select sport, click “Initial Registration, and click “Proceed”

  • Click “Start filling out a new form” for each of the 6 available forms.  Complete each form fully and click “Submit” at the bottom of the page for each of the 6 forms.  Click “Return to Registration” after submitting each form and proceed until all 6 forms show a status of “Completed”

  • ALL INITIAL REGISTRANTS (DID NOT PLAY A 2014 FALL SPORT) MUST COMPLETE ELECTRONIC FORMS:

  1. Interscholastic Sports Policy – with Parent and student electronic signature

  2. Permission for Participation – with Parent electronic signature

  3. NJSIAA Concussion Policy – with Parent and Student electronic signature

  4. NJSIAA Steroid Policy – with Parent and Student electronic signature

  5. Emergency Contact Information – with Parent electronic signature

  6. Sudden Cardiac Death Pamphlet Sign Off Sheet – with Parent and Student electronic signature

  • ALL PREVIOUS REGISTRANTS (PLAYED A 2014 FALL SPORT)

    • Must complete the “Previous Participant” packet which includes: Permission for Participation, Emergency Contact Information, and the Sudden Cardiac Death Pamphlet Sign Off Sheet.

New Jersey  PRE-PARTICIPATION PHYSICAL EXAMINATION and HEALTH HISTORY UPDATE QUESTIONNAIRE  

 
  1. Pre Participation Physical Examination (PPPE):  A physical examination that is less than 365 days old from the first day of the start of practice must be submitted.  Print out the FOUR page form found on either the SHSD.org website under the Athletic Link or the Health Office Link.  Parents/ Guardians and Athletes should complete and sign both the History Form (page 1) and the Athlete with Special Needs Supplemental Form (page 2).  Any yes answers on History Form need an explanation (in the box provided) with a date of the illness/injury.  Page 2 must be completed regardless of the answers.  Bring these completed forms to the participant’s physical examination appointment with your treating physician.  The physician should review the History and the Special Needs Supplemental form, perform the physical examination and then complete the Physical Examination (pg 3) and the Clearance (pg 4), making sure that all the elements of the physical examination are recorded. Treating physician should sign and stamp the form.  Please make sure that you fill in the date of the physical examination on the top of page 1, make sure all sections on all four pages are complete, and make sure physician stamp is next to the physician signature.  Incomplete forms will be returned.  Staple all forms together and submit to the red sports box in the main lobby at BHS between October 15th and October 29th.  Pre Participation Physical Examination (PPPE) forms can be found at:  http://www.state.nj.us/education/students/safety/health/records/athleticphysicalsform.pdf.

 
  1. Health History Update Questionnaire (HHUQ) Form:  If you have already submitted the four page PPPE that was completed  within 365 days of the start of practice but more than 90 days prior to the first day of official practice, you will ONLY need to provide a completed HHUQ that is signed by the student’s parent/guardian.  Any yes answers on the HHUQ should have treating physician documentation to substantiate a medcial  evaluation and medical clearance to return to PE/sports.  This form can be found at:  http://www.state.nj.us/education/students/safety/health/records/HealthHistoryUpdate.pdf.

ImPACT Baseline Concussion Testing

 

About Baseline Testing: The baseline test is one important tool used to diagnose a concussion or help evaluate when it is safe for an athlete to return to a sport after suffering a concussion.  If a concussion or possible concussion is sustained, it is likely that a post injury test will be administered at that time.  This test baseline test MUST be taken every two years, but can be taken annually for improved accuracy.

 

Instructions for Baseline Testing:

 

Allow for 30-40 minutes in a quiet and distraction free environment.

    *No cellphones, tv, music, or other distractions.

 

Try to use a computer with a mouse if possible.

 

Go to the website www.impacttestonline.com/schools

 

Select “New Jersey” from the dropdown box and click “Launch Baseline Test”.

 

Enter the code: B8AC4DCF8C

While holding down the control key, hit “Launch Baseline Test”.

    *If the test does not launch, then make sure that your pop-up blocker allows pop-ups from www.impacttest.com and retry.

 

Fill out the required demographic information.

 

Read all instructions carefully for each section of the test and begin when prompted.

 

Please print the confirmation after the test is completed and submit to Mr. Matt Bale-Pena, athletic trainer, in person or to his mailbox in the front office.

Transportation
Athletes are encouraged to ride the bus to and from from all athletic contests for all of the obvious team building reasons. However, we realize that it may be more convenient for parents to drive them home on OCCASION. Please send an email to my attention attention requesting this in advance. Be sure the coach knows who you are and sees that your son/daughter is with you at the conclusion of the contest. Remember that parents are ONLY allowed to transport their own children! The exception would be if they are BOE APPROVED team drivers. Those forms can be sent electronically upon request. Thanks!

Sportsmanship
If I may, a comment about officiating and sportsmanship. Good and bad officiating is part of all sports and at all levels. If professional officials cannot get it right all of time, it’s difficult to expect HS officials to get it right (all of the time). When they don’t, as a parent I know you want to scream, yell and voice your opinion. I’m asking that you refrain from doing this. It’s difficult enough to get officials to work our games and when parents lose control, it only makes matters worse. We’re the adults and for better or worse, it’s up to us as parents to set the example of appropriate behavior at athletic events. Former Acting Governor Richard Codey had legislation passed regarding fan behavior at athletic events. I think we do an outstanding job at BHS and I hope the positive behavior that we have exhibited in the past continues. Please review again the additional Sportsmanship information on the shsd web site.

Bus Transportation to Polo Grounds
Attention Parents and Athletes:

There will be two buses available after school to transport student athletes to the Polo Grounds for games and/or practices every day, if there are seats available. The buses will be the LAST TWO buses parked in the "bus line" in front of the school.
