ALL FALL SPORTS DOCUMENTATION IS DUE NO LATER THAN June 1, 2015

 

NOTE: TO RECEIVE APPROVAL TO PARTICIPATE IN A FALL SPORT, A MINIMUM OF TWO WEEKS FROM SUBMISSION OF ALL COMPLETED FORMS IS REQUIRED.

Fall sports practices will start on August 9, 2015.  For students to be eligible to participate in the first practice, the following 3 REQUIREMENTS must be completed between May 18 and June 1, 2015:

  1. Submit a Pre-Participation Physical Evaluation (PPPE) and/or Health History Update Questionnaire (HHUQ) to the either the Red Sports Box outside the BHS Main Office or the BHS Health Office.

  2. Complete electronic athletic forms.

  3. Submit the $75 co-curricular fee directly to Athletics Department or via RevTrak (click “Online Payments” on district website home page).  Co-curricular fees are collected once per academic year for participation in sports/clubs.

1. Submit a Pre-Participation Physical Evaluation (PPPE) AND/OR Health History Update Questionnaire (HHUQ): Print the 4 page PPPE from the shsd.org athletics webpage, or the link below.  Athletes, parents should complete

  • Page 1: (History Page) EACH “yes” answer requires a clear explanation with dates of illnesses/injury – list that information in the box located in the lower right hand corner.  Make sure date of physical exam is completed on top of the form. The physical exam date (completed on top portion of Page 1-History Page) must be within 365 days before first day of official practice.  If this date is 90 days before the start of practice you need to complete HHUQ (see details next paragraph).   Make sure athlete and parent sign and date bottom of form.  

  • Complete Page 2 if applicable.  

  • Bring completed page 1 and/or 2 to participant’s doctor for review.   Physician completed IN FULL Page 3 (Physical Examination Form) and Page 4 (Clearance Form).  Make sure a vision screening is documented on Page 3 (Physical Examination Form).

Health History Update Questionnaire (HHUQ): If a student’s completed physical  is less than 365 days old but more than 90 days old, fill out the HHUQ.  When completing this form, make note of the completed physical date and answer the questions appropriately.  Since your last physical, if you answer yes to any of the questions, please attach treating physician medical evaluation and medical clearance.

INCOMPLETE FORMS WILL NOT BE ACCEPTED AND WILL BE RETURNED VIA MAIL TO PARENT.

Submit COMPLETED FOUR PAGE PPPE and the HHUQ  to red box outside the BHS main office between May 18 and June 1, 2015. If the PPPE will be scheduled to be completed after June 1, 2015, please contact Mrs. Connor at aconnor@shsd.org and state such in an e-mail, along with a date of scheduled treating physician annual physical.  All PPPE’s must be submitted prior to July 15, 2015 in order to be cleared to participate in a fall sport.

 

2.Complete Electronic Sports Forms

  • Go to www.gotomysportsphysical.com

  • Register or log in with your e-mail address (one account per household)

  • ALL ATHLETES MUST COMPLETE AN ON-LINE REGISTRATION FOR EACH SEASON

  • Click “Fill Forms” – Complete all 6 forms, answer ALL questions, and include parent/student signatures as indicated

  • TWO WEEKS AFTER SUBMITTING FORMS, students can check their status on www.gotomysportsphysical.com.  “Complete and Stamped” means that ALL THREE REQUIREMENTS have been met for participation.  An e-mail is automatically sent to the household’s account e-mail from gotomysportsphysical.com when a student is approved.

 

3.Submit the $75 Co-curricular Fee

Submit checks payable to “BHS” to Mr. Hoppe’s office or electronically via RevTrak (click “Online Payments” on shsd.org).

 
 


 

 

 

ImPACT Baseline Concussion Testing

 

About Baseline Testing: The baseline test is one important tool used to diagnose a concussion or help evaluate when it is safe for an athlete to return to a sport after suffering a concussion.  If a concussion or possible concussion is sustained, it is likely that a post injury test will be administered at that time.  This test baseline test MUST be taken every two years, but can be taken annually for improved accuracy.

 

Instructions for Baseline Testing:

 

Allow for 30-40 minutes in a quiet and distraction free environment.

    *No cellphones, tv, music, or other distractions.

 

Try to use a computer with a mouse if possible.

 

Go to the website www.impacttestonline.com/schools

 

Select “New Jersey” from the dropdown box and click “Launch Baseline Test”.

 

Enter the code: B8AC4DCF8C

While holding down the control key, hit “Launch Baseline Test”.

    *If the test does not launch, then make sure that your pop-up blocker allows pop-ups from www.impacttest.com and retry.

 

Fill out the required demographic information.

 

Read all instructions carefully for each section of the test and begin when prompted.

 

Please print the confirmation after the test is completed and submit to Mr. Matt Bale-Pena, athletic trainer, in person or to his mailbox in the front office.

Transportation
Athletes are encouraged to ride the bus to and from from all athletic contests for all of the obvious team building reasons. However, we realize that it may be more convenient for parents to drive them home on OCCASION. Please send an email to my attention attention requesting this in advance. Be sure the coach knows who you are and sees that your son/daughter is with you at the conclusion of the contest. Remember that parents are ONLY allowed to transport their own children! The exception would be if they are BOE APPROVED team drivers. Those forms can be sent electronically upon request. Thanks!

Sportsmanship
If I may, a comment about officiating and sportsmanship. Good and bad officiating is part of all sports and at all levels. If professional officials cannot get it right all of time, it’s difficult to expect HS officials to get it right (all of the time). When they don’t, as a parent I know you want to scream, yell and voice your opinion. I’m asking that you refrain from doing this. It’s difficult enough to get officials to work our games and when parents lose control, it only makes matters worse. We’re the adults and for better or worse, it’s up to us as parents to set the example of appropriate behavior at athletic events. Former Acting Governor Richard Codey had legislation passed regarding fan behavior at athletic events. I think we do an outstanding job at BHS and I hope the positive behavior that we have exhibited in the past continues. Please review again the additional Sportsmanship information on the shsd web site.

Bus Transportation to Polo Grounds
Attention Parents and Athletes:

There will be two buses available after school to transport student athletes to the Polo Grounds for games and/or practices every day, if there are seats available. The buses will be the LAST TWO buses parked in the "bus line" in front of the school.
